Health Navigator (Ambulatory Care) helps members navigate and connect with clinical and community resources. Supports clinical staff in identifying and providing referral and baseline assessment services to members. Establish and maintain effective ongoing relationships by facilitating communication and coordination with members, their caregivers, PCPs and other providers. Connects members to health screening programs and other resources. Health Navigator focuses on care coordination and short-term case management for members who need help with medical, social or behavioral services.

You will focus on:
- Under the general supervision and direction of CM management, the Navigator supports clinical staff through the execution of case management and disease management program components.
- Help members make appointments and access relevant health and community programs.
- Provide members with personalized guidance, support, education, care coordination and other assistance as they navigate the health care system.
- Initiate follow-up to confirm and coordinate additional needs. Attend case conferences and meetings with the CM team and medical directors to support effective care coordination.
- Communicates effectively with team members, patients/families and physicians.
- Ability to provide a high level of customer service to members, internal and external customers in a professional and service oriented manner
- Educate and respond to questions from members and/or their guardians about benefits, services, eligibility and referrals in a positive and professional manner that promotes member satisfaction and member retention.
The skills, experience and education you bring with you:
- We expect a college or equivalent education, experience or training.
- Minimum Experience: Two (2) years in an acute care setting, preferably in case management or emergency department.
- Strong organizational skills. Strong verbal and written communication skills. Ability to effectively handle stressful situations, manage multiple and sometimes conflicting priorities simultaneously.
- Knowledge of managed care, including contracted and non-contracted providers.
- Fluent spoken and written English. Maintain the confidentiality of all PHI in accordance with state and federal law and BTP policy.
